MHC provides 24-hour supervision, services, and housing as well as residential and support services where we provide in-home care.
Mental Health Connecticut’s (MHC) residential and home-based services provide individualized supports for adults with mental health conditions. MHC’s home-based services can include the development of person-centered, goal-oriented, recovery plans, the development of goals to pursue community involvement and natural supports, skill building activities and residential supports to promote successful independent living, and engagement with family members.

The goal of life coaching services is to promote self-care skills by offering natural wellness and healing activities that focus on: Nutrition and healthy eating, Sleep hygiene, Meditation and mindfulness, Physical activity. Creating safe and supportive living environments, Fostering connection and a sense of belonging to improve mood and health. Life Coaching uses unique pathways to create a ripple effect of hope that result in action. Through this program, many achieve goals in acquiring new skills and make other changes that lead to gaining independence, embracing self-advocacy, decreasing symptoms of their mental health conditions, and rejoining the community in a meaningful way.
MHC offers supported education services through its Lifelong Learning Supported Education (LLSE) program, which are designed to provide the supports that people with mental health conditions may need to earn their high school diplomas, GEDs, certificates of learning and college degrees.
Through the waiver program, the State of Connecticut offers an array of home and community-based services. These services assist Medicaid eligible adults with serious mental health conditions with living successfully in the community and avoiding institutional care such as nursing homes. Mental Health Connecticut (MHC) is a state-approved Mental Health Waiver provider. We have diversion nurses who go out and go to the people in the nursing home and do a full assessment. They create a care plan to determine what it would take to have someone go out into the community with the services and support that they need. If it costs the same or less than the nursing program, MHC is able to get approval for the individuals to move into an apartment and receive services.
Mental Health Intensive offers a home with 24/7 supervised care for individuals with a serious mental health condition.
