The Mental Health Concierge Program is a first-of-its-kind service that brings personalized mental health and wellbeing support directly into people’s homes. Unlike traditional home companion care, this program blends compassionate presence with a structured, evidence-based care plan tailored to each individual’s needs.
Every participant is paired with a dedicated Community Support Specialist and Recovery Coach, and can access additional services such as vocational coaching and counseling. Using motivational interviewing and a holistic approach, our team helps people identify and overcome barriers to progress while building the skills, confidence, and resilience to thrive.
We serve individuals ages 16 and older, with or without a formal diagnosis, who are seeking personalized, in-home support to strengthen their mental health and overall wellbeing. Our services are especially beneficial for those transitioning home after hospitalization or treatment, helping to ensure continuity of care and a successful return to daily life.
We also support families and caregivers by providing consistent guidance, resources, and connection, and partner with providers and care teams as a trusted extension of aftercare and wraparound services. To ensure this level of care is appropriate and effective, clients must be able to safely be without services for at least three consecutive days.

You’ll be matched with a dedicated team committed to holistic support and seamless care coordination, ensuring every aspect of your care feels thoughtful, connected, and aligned with your goals. Our approach focuses on building practical skills and providing meaningful emotional support, creating a foundation for lifelong wellness.
With compassion at the core of everything we do, our team brings connection, consistency, and care directly to your doorstep.

The Recovery Coach offers in-home support with daily living, routines, coping strategies, and goal-setting to keep individuals moving forward in recovery.

The Vocational Coach helps individuals explore goals, build job skills, prepare for employment, and stay supported throughout the job search and workplace transition.

The Community Support Specialist provides seamless care coordination, connects individuals to services and resources, and ensures supports work together.

Meaningful connection through community supports, including MHC’s Art of Wellbeing for creative expression, connection, and coping strategies.

Evidence-based therapeutic support from licensed clinicians to address mental health needs and promote lasting emotional wellness.
Getting started is simple. Reach out to our team to begin the process, complete a comprehensive assessment so we can understand your unique needs and goals, and receive a personalized care plan with clear service recommendations and pricing options tailored to you. Our team will walk you through each step, ensuring you feel informed, supported, and confident in choosing the level of care that’s right for you.
The intake includes a full assessment and individualized recovery plan. We apply a $150 credit toward your starter package.
