Mental Health Connecticut (MHC) actively engages in the Northwest Coordinated Access Network (CAN), offering essential services like homelessness outreach, prevention, and diversion.
Our staff asses the housing needs of individuals experiencing homelessness to put together an action plan and provide resources. Staff are trained to explore an individual’s strengths to reconnect them to natural supports, communicate with landlords, and identify solutions to resolve or prevent housing crises.

Mental Health Connecticut participates in the Northwest Coordinated Access Network (CAN) that serves those who meet the HUD definition of literal homelessness, which is required for entry into homeless projects throughout the state. CANs assess the housing needs of all individuals experiencing homelessness. Staff are trained to explore an individual’s strengths to reconnect them to natural supports, communicate with landlords, and identify solutions to resolve or prevent housing crisis.

MHC is proud to serve as a provider for the Housing Empowering Recovery Opportunities (HERO) program, a statewide initiative funded by the Connecticut Opioid Settlement Advisory Council and led by the Department of Mental Health and Addiction Services (DMHAS).
This innovative program aims to bridge the gap between housing instability and recovery by supporting individuals who are experiencing or at risk of homelessness and are living with substance use disorders. MHC will deliver services in the Northwest region (Waterbury and Litchfield) alongside McCall Behavioral Health Network, and in Fairfield County with Recovery Network of Programs.
Interested individuals will be able to complete a screening form, which will reviewed and verified by DMHAS. Once eligibility is confirmed, in coordination with a licensed provider, clients will be connected with a local provider, including MHC. MHC is contracted by third-parties to provide case management to individuals with subsidized housing. Through this service, we are able to provide case management, helping with care coordination and recovery plans. We provide this service in Litchfield County.
The Homeless to Housing (H2H) program in Connecticut is a collaborative service delivery program that helps people experiencing unsheltered homelessness find and maintain stable housing. The program began on July 1, 2023, and is implemented by the Department of Mental Health and Addiction Services (DMHAS). Our CAN Navigators support H2H Housing Specialists and help them locate a shelter bed. They assist in connecting the individuals to medical care, behavioral healthcare, housing, employment, and benefits. They also assist in replacing required identification documentation and in linking them to additional supports such as insurance, SNAP, childcare, and education.
