Are you or someone in your care looking for more support at home or in your current living facility? Mental Health Concierge is an in-home wellness and care team that comes to you.  

Individuals, families, and conservators turn to Mental Health Concierge for peace of mind and confidence that in-home services are matching the right need at the right time. For those with private pay options, this unique service helps individuals with mental health conditions stay in their home and improve their quality of life through personalized, in-home wellness services. 

Those receiving Mental Health Concierge services are individuals who are living with a mental health condition, aging in place, Veterans, individuals with disabilities, and as support for the care givers and the family.  

By working with the individual’s behavioral health and primary care team, Mental Health Concierge supports the goals of the individual to enhance their overall quality of life, increase coping skills, reduce symptomatology, adopt new life skills, and decrease use of emergency services.  

By working with our Concierge team, individuals receive:

    • A thorough needs assessment
    • 3-month check-ins to reassess needs
    • Support from a team trained in evidence-based, person-centered care, as well as staff who have lived experience and peer support credentials
    • Whole health care plans designed to meet day-to-day needs and improve the quality of life for the individual and care givers
    • A collaborative approach to care with Full case management services and assistance for the individual and care givers with navigating the mental health system
    • Referrals to MHC partners that will complete the picture of support needed at the right time

Mental Health Concierge’s services are unique to each person’s needs and begin with a thorough understanding of how we can meet the individual where they are.  

Key areas of support include: 

Clinical Support: Collaborates with the individual’s behavioral health and primary care team to build a person-centered recovery plan. This can include performing initial assessments, identifying goals and objectives, and implementing person-centered strategies.

Community Support: Using a team approach, Mental Health Concierge provides rehabilitative, crisis intervention, education and skill building supports with the ultimate goal of promoting independent living and community integration. Services can also include engaging family members and assisting with monthly budgeting.

Recovery Coaching:
Provides life skills coaching, cuing and modeling in all areas of household chores, including cleaning, laundry, meal planning and preparation, and shopping for nutritious foods. Recovery coaching also helps individuals identify ways to engage in social and community activities and provides short-term, in-home respite as needed.

Vocational Coaching:
Provides ongoing supports to prepare individuals for competitive employment opportunities. Vocational Coaching may include assessing skills, identifying employment opportunities, educating the individual about interviewing techniques and work culture, and providing information and support related to self-advocacy on the job.
